Groundwater (GW) Rule Forms
Disinfection By-Products Phase 2 Rule - IDSE (initial distribution system evaluation) forms are required under the disinfection byproducts phase 2 rule. The four options of the IDSE are Very Small System Waiver, 40/30 certification, System Specific Study (form not included), and Standard Monitoring Form. Choose the correct form according to your system water type, ground water or surface water, and population.

Arsenic Exemption Request
All Community and Transient Noncommunity drinking water systems must comply with a new arsenic standard of 10 ppb by January 1, 2006. An exemption from this requirement may be available, however. To apply for this exemption complete this form and send it to the Division of Drinking Water.
Operation of Existing Water Treatment Facilities

- Monthly Electronic Report (xls) This is an electronic form for operators of complete treatment plants. It can be completed and e-mailed back to the Division
(updated 11/30/2011) - Disinfection Profiler Spreadsheet (xls) This spreadsheet calculates a disinfection profile for one month. Allows for up to 20 disinfection sequences. It can be used for the following disinfectants: chlorine; chloramines; chlorine dioxide; and ozone.
