Quick Start Guide
- All applicants must fill out a Citizenship/Alien ID Certification form.
- Fill the form out online using the latest version of Adobe Acrobat
- Print the Form for your own records and submit to DDW.
- Adobe Reader Users: fill out the form, hit the "Submit by Email" button, and then eMail the xml file to Rich Peterson (using the Wizard)
- Adobe Pro Users: fill out the form, save the pdf, and then eMail the pdf form to Margaret Hand
- Pay Online and print a copy of your receipt
- Optionally you can mail your completed form(s) along with a copy of your receipt to DDW at 195 N 1950 W SLC, Utah 84116
- If you have problems with any of the forms, contact the DDW webmaster or call (801) 536-4035 for help.

All applicants must fill out a Citizenship/Alien ID Certification form. Print out the form, sign and date it, have a public notary notarize the document, and then mail it into DDW at 195 N 1950 W SLC, Utah 84116. This is a one-time-only requirement.

Detailed Instructions
You may use our “Shopping Cart” system to submit application forms online and pay fees associated with those applications. The system will accept Visa, Master Card, American Express, Discover or you can pay by Virtual Check.
Currently, a few of the forms can be filled out “on line” (you may need the latest version of Acrobat Reader though). You can then either "Submit by Email" or print out the completed form and mail it to the Division. Some forms, however, must be printed out first and filled out manually. Once the form is complete, you then mail it to the Division.
When you mail the form, you must attach a copy of the receipt you obtained after going through our shopping cart payment system.
To illustrate, let’s assume that you want to take an upcoming operator certification exam. Browse the area above and find “Operator Certification Forms and Fees / Sign up to take an exam”. Note there is a PDF form associated with this item. Click the PDF form to open it. Fill out the form online using Acrobat Reader. After completing the form, hit the "Submit by Email" button near the top of the form. This will create an XML file and trigger your default eMail program to send an eMail with your XML file attached to DDW. Staff will then import the XML file into a blank pdf form and save it as a completed pdf form. We will then reply back to your eMail and attach the completed pdf form for your records. After "Submit by Email", you may click the "Pay Online" button (alternately you can click on the link from this page) this will take you to the area for operator certification exam payment. Click to add to cart and proceed to check out to pay by credit card or virtual check. If you choose to mail in your application, you must print and attach a copy of the payment receipt form as you go through the “Shopping Cart” payment system. The mailing address for the Division of Drinking Water is 195 North 1950 West Salt Lake City, Utah 84116.
Also as of July 1, 2009, all applicants must fill out a Citizenship/Alien ID Certification form and mail it into the Division.
